After installation I did not get any option or settings page in Dashboard, also I thought it kept as widget but alas no any widget of it.
waste of time..lol

This plugin should be removed from the repository. Not only is it old and not been updated but it is broken and incorrectly coded.



First, it defines ABSPATH using the wrong path which fortunately fails because it should already be defined and cannot be defined twice, but this still causes a warning in your PHP error_log files on every page load.



Then , if you use the shortcode on a page or post you get a broken Google Map with the error: "This page can't load Google Maps correctly." and each of the form fields fails to function with that same error message in a popup box when you try to enter anything at all.
