Displays a contact form which will post user submitted information into a VICIdial system utilizing the non-agent API and call them.

<p>Uses a custom shortcode to display a contact form that will submit data to the configured VICIdial system as a new lead and into the hopper with a priority of 99 via the “add_lead” non-agent API function. Currently allows for first name, last name, phone number and comments to be submitted.</p>
