This is a generic Courier Address field management plugin with special extensions for Courier delivery charging. It is compatable with <a href="https://wordpress.org/plugins/contact-form-7/" rel="ugc">Contact Form 7</a> but also creates generic form element shortcodes. <a href="http://wordpress.xsearchservices.com/allplugins/courier-address/" rel="nofollow ugc">Working example</a>
