We've used a number of different form plugins over the years and this one is definitely top notch with tons of customization options. Very flexible and easy to use.

Really an incredibly advanced form builder that you can pretty much do anything with, no matter how simple or complex. 



Support (I'm using a paid version) is really outstanding, they respond quickly and have useful answers and help,
