Installation

Download the plugin files and save them in a folder named frontend-form-with-csv-export.
Upload the folder to the wp-content/plugins/ directory of your WordPress installation.
Activate the plugin from the WordPress Plugins menu.
Add [pdavea_frontend_form] shortcode to any post, page, or widget.
