Put the plug-in folder WP Smart Preloader into [wordpress_dir]/wp-content/plugins/
Go into the WordPress admin interface and activate the plugin
Go to settings => WP Smart Preloader
Choose the Options of your choice
Click Save changes button.

It is not a CSS Only Preloader (as advertised)

It will cause a blank page to be displayed if Javascript is turned off.
