Clicking to flag a post or page will present the visitor with a popup modal (lightbox) form. You can determine what information the user must provide in order to submit the flag.</p>

It is the only one plugin, that I tested and it works properly. Also with themes depend on colour of the site :) I use this for getting informed, via users, about taken off embeded Youtube videos. Thanks a lot!
