Installation

Download repo’s tarball/zip and extract it to your WordPress’s Plugins folder ({wordpress-folder}/wp-content/plugins)
Enter your WordPress Administrator panel (http://my-wordpress.url/wp-admin)
Activate plugin under Plugins section
Go to post/page you want to embed your form
Enter your form URL to your blog post (Ex: http://www.jotform.com/form/987654321)
Save/Preview it
Hola, your form is embedded to your post/page 🙂
